To minimize time in the office, if possible, we ask all new patients to complete the online web registration or print and fill out the three patient registration forms before coming to the office.
If you are feeling ill or have travelled outside the country in the last two weeks, please call to reschedule your appointment.
Our office will be cleaned regularly and sanitized between patients.
